An ODFI is the Originating Depository Financial Institution, which originates and submits ACH transactions. An RDFI is the Receiving Depository Financial Institution, which receives and processes them. Look up any ACH reject code below for what it means and what to do about it.

| R04 - Invalid Account Number | The account number provided with a transaction is incorrect because the number has incorrect digits, doesn't pass digital validation, or doesn't match the account numbers at the RDFI. | Validate account numbers against a predefined structure using a service such as Plaid or GIACT. | Do not resubmit. This Payment Profile is permanently disabled; have the customer set up a new payment method with the correct account number. |
| R07 - Authorization Revoked by Customer | The customer has revoked the authorization of the transaction. | Keep updated authorizations and communicate with customers. | Do not resubmit. This Payment Profile is permanently disabled; the customer needs to set up a new payment method with a new written authorization. |
| R08 - Payment Stopped | The account holder has placed a stop payment order on this specific transaction. This code is the general order for most stopped payments, with the exception of stops on a source document or RCK entries (Re-presented Check Entry, or bounced checks), which have specific codes below. | Confirm authorization and payment details with the account holder. | Contact the account holder to obtain a bank letter to provide to Maxio to reprocess. |
| R09 - Uncollected Funds | The account may have deposits that have not yet cleared, resulting in insufficient funds for the transaction. | Be aware of deposit clearing timelines. | You may resubmit once the customer confirms the balance is available. |
| R10 - Customer Advises Not Authorized | The customer has stated the transaction was not authorized. | Secure proper authorization before initiating the transaction. | Do not resubmit. This Payment Profile is permanently disabled; obtain new written authorization and have the customer set up a new payment method. |
| R12 - Account Sold to Another RDFI | The customer changed banks and the account has been transferred to another RDFI. | Update account information regularly. | Obtain the new bank details from the customer and update your records. |
| R13 - Invalid ACH Routing Number | The ACH routing number is incorrect. | Verify routing numbers before initiating transactions. | Do not resubmit. This Payment Profile is permanently disabled; have the customer set up a new payment method with the correct routing number. |
| R14 - Representative Payee Deceased | The representative payee is deceased or cannot perform duties. | Keep track of representative payees and their status. | Do not resubmit. This Payment Profile is permanently disabled; the new representative payee needs to set up a new payment method. |
| R15 - Beneficiary or Account Holder Deceased | The beneficiary or account holder is deceased. | Regularly update account holder and beneficiary information. | Cease transactions and consult with the estate or new account holder. This Payment Profile is permanently disabled. |
| R16 - Account Frozen | The account is frozen, due to legal action or bank policy. | Keep track of legal issues related to accounts. | Consider the risks involved with continued payment from customer via ACH, and contact the customer for resolution. This Payment Profile is permanently disabled. |
| R20 - Non-Transaction Account | Policies or regulations prevent ACH transactions on this account. | Confirm account types before initiating transactions. | Use an alternative payment method or account. This Payment Profile is permanently disabled and can't process ACH. |
| R21 - Invalid Company Identification | Company ID information is incorrect or outdated, commonly caused by a miskeyed entry. | Validate company IDs before initiating transactions. | Update the company ID and resubmit. |
| R24 - Duplicate Entry | The same transaction was submitted more than once. | Implement controls to prevent duplicate transactions. | Confirm whether it's a true duplicate and take appropriate measures. |
| R29 - Corporate Customer Advises Not Authorized | The corporate account holder has notified the RDFI that the transaction is not authorized. | Secure proper authorization before initiating the transaction. | Do not reattempt. This Payment Profile is disabled, and reinstating it requires a Maxio Risk team review. See Resolve Blocked ACH Payments Due to R29 Return Code for the full process. |
| R45 - Invalid Individual Name | Account holder's name is incorrect or misspelled. | Validate names before initiating transactions using a service such as Plaid or GIACT, or obtain a voided check to validate the account name during customer setup. | Update the name and resubmit. |
| R82 - Invalid Foreign RDFI Identification | The foreign RDFI's identification is incorrect. | Double-check identification details of the foreign institution. | Correct the identification information and reprocess the transaction. |
| R83 - Foreign RDFI Unable to Settle | The foreign RDFI cannot complete the transaction. | Confirm the foreign institution's ability to settle transactions before initiating one. | Work directly with the foreign institution to resolve the issue. |
| R84 - Entry Not Processed by Gateway | The entry hasn't been processed by the designated gateway, a service provider that acts as an entry point to the ACH network for financial institutions. | Route transactions through a functional gateway. | Resubmit the transaction through a valid gateway, or contact the gateway for resolution. |
| R85 - Incorrectly Coded Outbound International Payment | An outbound international payment has been incorrectly coded. | Verify the coding of international payments. | Correct the code and resubmit the transaction. |
For R12, R21, R24, R45, R82, R83, R84, and R85, the disabled Payment Profile isn't permanent: a user with Customer Support access on the Merchant's account can re-enable it once the underlying issue is corrected, without needing to contact Maxio.
