Maxio Payments is built on Adyen, the payment processor that underwrites and processes your transactions. Applying for the Maxio Payments gateway means completing Adyen's own hosted application, which collects company, decision-maker, and payout details. Fields marked (Required) must be completed before you can submit the application.
This section collects your company's legal identity, structure, registration details, and address.
Provide your company's legal name and the country where it's registered.

| Country | (Required) Defaults to United States. If you need a different country, Adyen provides links to submit applications for other countries. |
| Legal name of company | (Required) Enter your company's legal name exactly as it appears on your Secretary of State registration. |
(Required) Select your company's structure:
If you select Governmental organization, you'll also need to identify who holds the bank account for your payouts: your company or an individual.
These fields confirm your company's registered business name and tax identification.

Select Manual entry, then start typing your address. Once it populates, select the address to fill in the fields below.
The application's summary page lets you edit any section if there are errors before you submit the application.
(Required) At least one Controlling Person and one Signatory. Any owner with 25% or more ownership must also be added.
Provide personal identifying information for each decision maker you add.

Each decision maker you add must meet the criteria for one of the following roles.
| Who | Criteria | Required number |
|---|---|---|
| UBOs through ownership | Individuals who directly or indirectly own 25% or more of the total shares, voting rights, or other equity in the organization. | Between zero and four. Adyen requires information about everyone who meets this criteria. If no one owns 25% or more, you must instead provide information about UBOs through control. |
| UBOs through control | Anyone who exercises ultimate effective control over decisions for the organization. If no such individual exists, members of senior management must be identified as UBOs through control. | At least one. Required if no one meets the UBO through ownership criteria, or if the organization operates in the United States. |
| Signatories | Officers or representatives who legally represent the organization to Adyen and are authorized to enter a binding agreement with Adyen on the organization's behalf. | At least one. |
| Directors | Only applicable if the legal entity is located in Australia, Canada, Hong Kong, New Zealand, or Singapore. A person responsible, individually or as part of a board, for the organization's overall governance and strategic direction. | Every director of the company. |
Your payout details identify the account that receives funds from transactions processed on the Maxio Payments gateway.
